LCES Kindergarten Registration

Welcome to Kindergarten!

If your child has turned 5 on or before August 31st, 2024, you may register them for Kindergarten!  On-line enrollment is available at lacenterschools.org or at the elementary office from 9:00 a.m.-2:00 p.m.  Here is some information to help guide you through the registration process:

OPEN REGISTRATION STARTS APRIL 8th – 19th

We are unable to accept registrations BEFORE APRIL 8th 

We require the following:

Legal, state-issued birth certificate

Residency Verification Documentation

Home Language Survey

Student Health History

Family Emergency Plan

Current Immunization Record

All of these forms can be found under New/Returning Student Registration on our website:

www.lacenterschools.org

 Immunization records must be turned in to the school office on or before the first day of attendance. 

Shot records must be from a health care provider, or you must attach paperwork from a health care provider to your handwritten form that shows your child’s records are accurate.  If your child was immunized in the state of Washington, the office can also print a current Certificate of Immunization form.

Your child cannot attend school until you provide these records.

If you choose to exempt your child from immunizations, a Certificate of Exemption form is available from the office and requires a doctor’s signature.

If your child has a life-threatening health condition (asthma, allergies, etc.) requiring medication at school, please contact our nurse, Danielle Rivers, at 360-263-2134, ext. 2118 or [email protected]  for important information.

Your child will be assessed here at the school either May 30th from 8:00 a.m. – 1:30 p.m. or May 31st 8:00 a.m. – 11:00 a.m.  The information gathered helps teachers find the best possible placement for your child.  Assessments can be scheduled online using SignUp.com.

For children that have completed the assessment, you will receive an email over the summer regarding class placement, along with an appointment time for a meeting called ‘Family Connection’.  This meeting is an opportunity for the teacher to get to know you and your child a little better!  If you are unable to make the scheduled assessment on May 30th or 31st, your child will have to wait until school starts to determine class placement.

Family Connection meetings (WA Kids) are estimated to take about 25 minutes and will be held Wednesday, August 28th, Thursday, August 29th and Friday, August 30th.  Teachers will schedule your appointment times.

Your child may bring their school supplies to this meeting.

Your child may attend either a morning or afternoon session.

9:00 a.m. –11:00 a.m.   OR   12:00 p.m. –2:00 p.m.

Expect an email invitation from your teacher by the end of June.

Kindy Smart Start is a time for students to become familiar with the school, the kindergarten teachers and make new friends!  Smart Start will be located in our Kindergarten classrooms.  

First day of School for Kindergarten:

*Teacher assignments may change through the first week of school based on increasing enrollment.

Immunization Requirement Information

Dear Parent or Guardian,

For all newly enrolled students, all immunization records turned in to schools are required by state law to be medically verified. Immunization records must be turned into the school on or before the first day of attendance. This means immunization records turned in to the school must be from a health care provider, or you must attach paperwork from a health care provider to your handwritten form that shows your child’s records are accurate. Your child cannot attend school until you provide these records.

Here are some examples of medically verified immunization records:
• A completed Certificate of Immunization Status (CIS) signed by a health care provider. Find the CIS form by visiting https://www.doh.wa.gov/SCCI and clicking on “Certificate of Immunization Status.”
• A CIS filled out by you or another parent/guardian with medical records attached.
• A CIS printed by a health care provider or school from the Washington State Immunization Information System.
• A CIS printed from MyIR which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go to https://wa.myir.net/register to begin the sign-up process.

If the printed CIS from the school or MyIR is missing any required immunizations for school attendance, medical records will need to be provided to the school office.
If you are requesting an exemption from one or more of the immunization requirements, you must provide the school a completed Certificate of Exemption.

myIR - My Immunization Record

What is MyIR?

MyIR stands for “My Immunization Record” and it’s an electronic portal for consumers to directly access their official state immunization records online.  In the past, consumers could only get their immunization records by calling or visiting a healthcare provider or their child’s school or contacting the state health department. This process can be time-consuming and burdensome for providers, patients, school staff, and state health departments, particularly during kindergarten registration, back-to-school season, or disease outbreaks when providers receive a high volume of immunization record requests. Registering for MyIR gives consumers a secure, easy method for accessing their and their dependents’ immunization records in real time.
